What to bring to your first visit

Your first appointment is a conversation about your sleep and your health. Bringing a few things along helps your physician get the full picture and keeps check-in quick.

  • 1 A valid photo ID
  • 2 Your current insurance card
  • 3 A list of all medications & doses
  • 4 A referral, if your plan requires one
  • 5 Notes on your sleep symptoms
  • 6 Prior sleep study results, if any

Your intake forms

We handle new-patient paperwork through our secure online patient portal, so there's no clipboard waiting for you in the lobby. After you book, you'll receive an email invitation to complete your intake forms from home, on any device.

Complete forms online

Your health history, symptoms, and consent forms are all filled out in the portal — usually about ten minutes, on your own schedule.

Secure & private

The portal is encrypted and HIPAA-compliant. Your information goes straight to your care team — never over email or paper.

Ready to get started? Book your appointment and your intake link arrives by email. Already a patient? Sign in to the portal to update your details.

Please try to finish your forms at least 24 hours before your visit so your physician has time to review them. If you'd rather complete them in person, arrive about 15 minutes early and we'll help you.

Prefer to print? Download your forms

The online portal is the easiest route, but you're welcome to download these to review or fill out ahead of time. Bring completed forms to your visit or finish them in the portal — please don't email medical forms.

Preparing for a sleep study

If your physician orders a sleep study — whether it's an easy take-home test or an overnight stay in our accredited lab — a few simple steps help us capture the most accurate picture of your sleep.

Skip caffeine & alcohol that dayAvoid coffee, tea, soda, energy drinks, and alcohol on the day of your study, since they change how you sleep.
Keep your normal routineEat your usual meals and take your regular medications unless your physician tells you otherwise. Don't nap during the day.
Come clean & product-freeShower and wash your hair, but skip lotions, oils, hair gel, and makeup so the sensors stay in place.
Pack comfortable sleepwearBring loose, two-piece pajamas and anything from home that helps you rest — your own pillow, a book, reading glasses.
Bring your essentialsToiletries, any nighttime medications, and your morning routine items if you'll head out afterward. Your CPAP too, if you already use one.

Doing a home sleep test instead? You'll get simple, step-by-step instructions with the device — the same guidance applies: no caffeine or alcohol that day, and stick to your normal bedtime.

How do I get my intake forms?

Once you book, we'll email you an invitation to our secure patient portal, where you can complete every form from home. There's nothing to print — just click the link and follow the prompts.

Do I need a referral to be seen?

It depends on your insurance plan. Some plans require a referral from your primary care doctor and some don't. If you're not sure, call us at (602) 206-6262 and we'll verify what your plan needs before your visit.

What should I wear and bring for an overnight sleep study?

Pack comfortable two-piece pajamas, your toiletries, and any nighttime medications. Feel free to bring your own pillow or a book. Please skip caffeine and alcohol that day, and come with clean, product-free hair and skin so the sensors stay put.

Can I fill out my forms in the office instead?

Of course. If you'd prefer paper or need a hand, just arrive about 15 minutes early and our front-desk team will help you complete everything before your visit.

What if I already use a CPAP machine?

Bring it with you to appointments and to any in-lab study, along with your mask. Seeing your current setup helps your physician fine-tune your therapy.
